Issue is, it's not just the looks, but also:

people just feel dysphoric about it in general,
some people would like to (primarily) be penetrated, but anal sex is too painful for them,
even then, it needs preparation, unless you're a special kind of pervert that likes the fact, that people poop from there,
genital preferences of others.
